Instructions

 Download the blank form, print it, and fill it out in ink, by hand. Fill it out completely and it must include your signature.

You can scan and email the form and any necessary documentation to expedite your request to: 880unionyes@ufcwlocal880.com. Or, you can send it back to the Union via mail, or hand-deliver it with the required documentation to:
